Preparing for Your First Visit
We look forward to welcoming you to our orthopedic practice. To ensure your first visit goes smoothly, please follow these instructions:
Complete Your Paperwork: Sign up for MyChart and fill out any necessary forms in advance. This will help streamline your visit and reduce waiting time.
Bring Necessary Documents: Please bring your insurance card, a valid photo ID, and any relevant medical records, including previous X-rays, MRIs, or other imaging studies related to your condition. If you have had prior surgery on the joint or body part of interest, please request a copy of your operative report from your surgeon’s office and bring it to your visit.
List of Medications: Bring a current list of all medications you are taking, including dosages and frequency.
Arrival Time: Arrive at least 15 minutes before your scheduled appointment to allow time for check-in and any additional paperwork.
Prepare Questions: Write down any questions or concerns you may have so that we can address them during your visit.
